Funeral Insurance (Econo) Application

Econo New Policy Acceptance Form Explained
STEP 1:
- Choose the applicable block:
 
NEW Policy – HlalaNathi will provide the new policy number to you.
or
- Changes (Alteration) to existing policy – If you are making changes to an existing policy, please add the policy number to the block provided.
 
STEP 2:
- Complete sections 1, 2 and 3
. 
COVER means either R10 000 or R20 000.
PREMIUM please refer to Annex A and specify the correct premium for your needs.
PLEASE ALSO NOTE THAT:
- If the Policy is for you and your Spouse, the oldest one between the two of you should always be put down as the Main Member when completing the form.
 
- The Underwriter will always use your “age of next birthday” as your actual age when applying for a new policy.
 
- Meaning, if you are currently 64 years of age, the Underwriter will classify you as 65 on the policy, which means your premium will be set as an Overaged premium which is R158 per month and not the normal R79 or R80 premium.
 
STEP 3:
- Complete section 4.
 
- Extended family members mean e.g. brothers, sisters, mother, father, children over 21 etc.
 - Up to 4 people are allowed.
 - Nominated Beneficiary: Supply a name of a person that will receive the benefit in the case that the Main Member and Spouse should pass away. This person should be over 21 years of age, have an ID book and an active bank account.
 - If the Main Member passes away before the Spouse, the benefit will automatically go to the Spouse and vice versa.
 
STEP 4:
- Complete section 5 and sign the form.
 
STEP 5:
- Please read the Terms and Conditions on the next page and sign the form.
 
STEP 6:
- Please read and sign the POPIA page on last page.
 
Funeral Policy Terms and Conditions
- Policies must be paid on or before the 7th of each month.
 - Policies that are not paid by the 7th of each month will show as arrears.
 - If there is an arrears amount on your policy, no claim will be paid out.
 - If your policy is in arrears for more than 2 months, you will have to join again and a new joining fee will have to be paid, and the society will go into a 6 month trial period.
 - The 6 months waiting period is six full calendar months.
 - All claims will be paid within 60 hours (3 days) to HlalaNathi Funerals cc.
 - During the first 6 months after the commencement of deductions or payments, the member, spouse and children and other dependants will only enjoy cover in the event of death due to natural causes.
 
Documents needed to process a claim
- Certified copy of the death certificate.
 - Certified copy of the deceased ID document.
 - Certified copy of the main member’s ID document.
 - Certified copy of the bank statement.
 - Certified copy of beneficiary’s ID document.
 - Affidavit form signed by beneficiary.
 - In cases where a member was previously underwritten, a letter from that insurer stating the deceased name, ID and last one to six months payment details must accompany the claim.
 - Certified letter from the beneficiary if the money is to be paid to any other person.
 - BI-1663 register of death (Notification). In the event of unnatural death, a police report will be required.
