This will either be the beneficiary stated on the Application form or the Funeral Undertaker conducting the funeral i.e. HlalaNathi Funerals cc. name.
- Relationship to deceased:
Depends if it is a person or the Undertaker. If it is a person, you will write e.g. Brother, Mother, Sister etc. If The funeral will be done by us, you will write Undertaker.
Please tick the Electronic Funds Transfer box and complete all the bank details.

- If we (HlalaNathi) will be doing the funeral, our bank details will be filled in here.
